I think your header filter is the problem.

After I add your header filter and try to go to http://www.youtube.com/watch?v=QguSXtGBsC4 , I get a 404 error or 'url not found' from Google (Google owns Youtube).

I think Youtube needs to know the correct host header to serve the files. Your filter hides it.
Did it work before you added the host header filter?

My point was that just adding the ip does not enable the 'external' proxy. I do not think your Proxomitron has been using a proxy.
